Meets Hope
Integrated Medical, Behavioral, and Housing Services
for Every Neighbor in Need
Our Mission
At Second Chance Community Health, we believe that healing begins with stability.
We integrate medical care, behavioral health, and housing access under one compassionate umbrella — ensuring every individual, regardless of income or history, has a path toward wellness and belonging.
“Founded in Nevada, Second Chance is a nonprofit healthcare organization bridging the gaps between clinical care and community life — a model of whole-person, whole-community health.”
Three Pathways. One Heart.
Together, these pathways create a continuous circle of care — addressing both the medical and social determinants of health.
