About Second Chance

Mission, Governance, and Integrated Care Philosophy

Founded on the principle that every individual deserves access to comprehensive, compassionate healthcare — regardless of income, housing status, or history — Second Chance Community Health operates as a nonprofit organization committed to addressing both medical and social determinants of health.

Our Mission

Delivering integrated healthcare that treats the whole person, not just the diagnosis.

“To provide accessible, integrated medical, behavioral health, and housing services that empower individuals experiencing homelessness and underserved populations to achieve stability, wellness, and belonging.”

We believe healing begins with stability. By addressing medical needs alongside housing, mental health, and social support, we create pathways to sustainable wellness that honor the dignity and potential of every person we serve.

Compassion

We approach every individual with empathy, respect, and understanding, recognizing that trauma and hardship require trauma-informed, person-centered care.

Equity

Healthcare is a fundamental right. We remove barriers to access through sliding-scale fees, language services, and care delivery where people are.

Integration

Medical, behavioral, and social services work together seamlessly, ensuring coordinated care plans that address root causes, not just symptoms.

Integrated Care Philosophy

Our model recognizes that health cannot be separated from housing, mental wellness, or social stability.

Why Integration Matters

Traditional Healthcare Gaps

Conventional systems often treat physical health in isolation, leaving patients to navigate fragmented mental health, housing, and social services on their own — creating barriers that prevent sustainable recovery.

Our Integrated Approach

We co-locate medical providers, licensed therapists, case managers, and housing specialists under one umbrella, with shared electronic health records and unified care planning — ensuring every team member sees the full picture.

Evidence-Based Outcomes

Research shows integrated care reduces emergency room visits by up to 85%, increases medication adherence, and improves long-term housing stability — because we address the social determinants that drive health crises.

Three Connected Pathways

Housing & Stability

Transitional housing, permanent supportive housing placement, case management, and benefits enrollment.

Behavioral & Mental Health

Individual therapy, group counseling, medication management, substance use treatment, and peer support.

Primary & Preventive Care

Family medicine, chronic disease management, preventive screenings, lab services, and pharmacy access.

How It Works in Practice
