You’re Welcome Here
No Matter Your Story, Your Situation, or Your Starting Point
At Second Chance Community Health, we believe healthcare is a human right — not something you have to earn. Whether you’re experiencing homelessness, navigating the justice system, rebuilding after trauma, or simply don’t have insurance, our doors are open to you.
“We see your strength, not your struggles. We honor your journey, not your obstacles.”
— The Second Chance Care Team
