We welcome discussions with healthcare education programs seeking integrated care clinical training sites. Our model provides students with hands-on experience in addressing social determinants of health alongside medical and behavioral care.
Programs Supported

Nursing Rotations
BSN and ADN clinical rotations in integrated primary care, community health nursing, and population health management.
Hours Required:
- Community Health Nursing: 120 clinical hours
- Leadership/Management: 80 clinical hours
- Practicum/Capstone: 150-200 hours
Supervision Structure:
- Direct supervision by BSN-prepared RN preceptors
- Weekly case review with interdisciplinary team
- Exposure to integrated behavioral health model
- Population health data analysis projects
Transition-to-Employment Pathway: Strong clinical performance may lead to RN employment offers. Eligible for Workforce Fellowship if hired at 32+ hours/week.
Physician Assistant Clinical Placements
Family medicine and behavioral health rotations for PA students seeking community health experience.
Hours Required:
- Family Medicine Rotation: 5 weeks (200 hours)
- Behavioral Health Elective: 4 weeks (160 hours)
- Underserved Populations Elective: 4 weeks (160 hours)
Supervision Structure:
- Direct supervision by licensed PA or physician
- Daily patient encounters with immediate feedback
- Participation in interdisciplinary care conferences
- Warm handoff training with behavioral health
Transition-to-Employment Pathway: Outstanding PA students may be offered employment upon graduation and licensure. HRSA NHSC eligible site. Workforce Fellowship available.
Medical Student Rotations
Third and fourth-year medical student clerkships in family medicine and psychiatry within an FQHC model.
Hours Required:
- Family Medicine Clerkship: 4-6 weeks
- Psychiatry Rotation: 4 weeks
- Elective Rotations: 2-4 weeks
Supervision Structure:
- Direct supervision by board-certified physicians
- Exposure to FQHC operations and value-based care
- Integrated behavioral health consultation experience
- Social determinants of health case management
Transition-to-Employment Pathway: Residency-trained physicians may be considered for employment. HRSA NHSC loan repayment eligible site.
Behavioral Health Internships
Pre-doctoral psychology internships and post-doctoral residencies in integrated behavioral health.
Hours Required:
- Pre-Doctoral Internship: 2,000 hours (1 year full-time)
- Post-Doctoral Residency: 1,500 hours (1 year)
- Practicum Students: 300-600 hours
Supervision Structure:
- Licensed psychologist supervision (2 hours/week individual)
- Group supervision and case consultation
- Integrated care model training (PCBH/CoCM)
- Evidence-based practice implementation
Transition-to-Employment Pathway: Post-docs and licensed psychologists may transition to permanent positions. HRSA NHSC eligible. Workforce Fellowship available for qualifying roles.
Medical Assistant Training
Externships for medical assistant students seeking certification and real-world clinical experience.
Hours Required:
- Clinical Externship: 160 hours minimum
- Extended Placement: Up to 240 hours available
- Schedule: 3-4 days/week (flexible)
Supervision Structure:
- Supervision by RN or certified medical assistant
- Clinical skills competency validation
- EHR documentation training
- Patient-centered communication skills
Transition-to-Employment Pathway: High-performing externs are frequently hired upon certification. Workforce Fellowship may be available depending on role and hours.

Credentialed social work supervisors for MSW students and associate-level clinicians

Weekly team meetings integrating medical, behavioral, and social perspectives
Evidence-Based Practice Training
Structured curriculum in trauma-informed care, motivational interviewing, and integrated care models
Performance Milestone Reviews
Regular competency assessments aligned with program learning objectives
Paid Employment During Internship
Supervision is included as part of paid employment during internship. Many of our advanced-level practicum students and interns are hired as W2 employees, receiving both clinical training and compensation while accruing required supervision hours.
This employment-integrated model allows students to gain real-world experience, receive professional supervision, earn income, and qualify for our Workforce Fellowship program — all while completing their academic requirements.
