Housing
Second Chance helps individuals and families who are experiencing or at risk of homelessness find and keep stable housing. Housing is one part of our full-circle care model, alongside primary care, behavioral health, and case management.
Whole-Team Care
Every housing participant receives care from our whole team: primary care, behavioral health, and case management. That wraparound support is what helps people get housed and stay housed.
Space Is Limited
Housing placement depends on availability and is not guaranteed. An intake lets us assess your needs and connect you with resources, whether or not a housing spot is open.
Insurance-Based Care
Our services are billed to your health insurance. Bring your insurance card to intake and we will review your coverage with you.
Eligibility & What to Expect
Every housing referral starts with an intake at our intake hub, so we can make sure the program is the right fit for you.
Who Can Apply
- Experiencing homelessness or housing instability
- Living in Nevada or able to relocate to Nevada
- 18 or older (families with children welcome)
- Willing to receive care from all of our providers: primary care, behavioral health, and case management
- Health insurance coverage (we review it with you at intake)
Priority consideration: Veterans, chronically homeless individuals, families with children, people exiting institutions, and survivors of domestic violence.
We do not turn anyone away based on criminal history or substance use history.
What Participation Looks Like
- Meet with each of your care team providers
- Keep scheduled appointments and work toward the goals you set with your case manager
- Stay connected with your care team for as long as you are in housing

Assessment
Meet with our team to review your housing history, health needs, insurance, and goals. This is how we make sure the program is a good fit.
Begin Care With Your Team
If the program is a good fit, you start care with primary care, behavioral health, and case management.
Housing Placement
When a housing spot opens, your case manager works with you on placement.

- Photo ID, if you have one

- A list of your current medications

Primary & Preventive Care
Access onsite medical services including routine checkups, chronic disease management, and preventive screenings.

While no documents are required to receive services, having certain items can help us connect you to additional resources and benefits more quickly.
Helpful to Have
Photo Identificatio
Driver’s license, state ID, passport, or tribal ID helps with housing applications and benefit enrollment.
Social Security Card
Needed for employment, benefits applications, and some housing programs.
Proof of Income
Pay stubs, benefit statements, or tax returns help determine eligibility for financial assistance.
Medical Records
Current medications, diagnosis information, and insurance cards help coordinate your healthcare.
Veteran Documentatio
DD-214 or VA benefits information unlocks additional veteran-specific housing resources.
Custody Documents
For families with children, custody papers or birth certificates may be helpful for certain programs.

State ID Assistance
We’ll help you get a Nevada state ID, including covering fees if needed.
Birth Certificate Requests
We can order birth certificates from any state and help with processing fees.
Social Security Card Replacement
Assistance with Social Security office appointments and applications.
Benefits Application Support
Help applying for Medicaid, SNAP, SSI/SSDI, and other benefits programs.
Legal Document Recovery
Connections to legal aid for custody documents, divorce papers, and more.
